Virtual World Games with Pregnancy & Family Mechanics

No mainstream virtual world centers entirely on realistic pregnancy simulation, but several games integrate family planning, pregnancy events, and child-rearing into broader life-simulation gameplay.

Life Simulators (Deep Systems)

  • The Sims 4 (Base Game Free): "Try for Baby" → trimesters → birth event → full infant-to-elder lifespan. Mods (WickedWhims, Basemental) add realism.
  • Stardew Valley: Spouse pregnancy event → baby → toddler (2 kids max). Co-op multiplayer supports family play.

MMORPGs with Family Systems

  • Black Desert Online: Marriage → children who become workers/characters. Complex but grind-heavy.
  • Final Fantasy XIV: "Ceremony of Eternal Bonding" (marriage). Children appear in lore/cutscenes, not as persistent gameplay mechanics.

Mobile / Social Sims

  • Avakin Life / IMVU: Pregnancy clothing/animations via user-created content; no mechanical simulation.
  • BitLife (Mobile/Web): Text-based life sim. Pregnancy is a major random/event-driven mechanic with choices (keep, adopt, abort). Generational play.

Safety Note

User-generated content in social worlds (Avakin, IMVU, VRChat, Second Life) varies wildly in maturity. Always review ESRB/PEGI ratings and community guidelines. Use parental controls for younger players.

For a dedicated "pregnancy sim" experience, BitLife (free, text) or The Sims 4 (free base, deep mods) are your best bets.
