PS3 Menu Icons Missing? Fix XMB Icon Corruption Step by Step

This is a known "XMB icon corruption" issue. Try these fixes in order:

1. Hard Restart

Hold power button 10 seconds → beep → off. Unplug 30 seconds. Reboot.

2. Check Video Output

• HDMI handshake failure can push icons off-screen. Hold power 5 seconds on boot (second beep) → "Reset Video Output."
• Try Component/AV cable if HDMI is flaky.

3. Rebuild Database (Safe Mode) – Fixes 80% of Cases

1. Power off → hold power until 2 beeps (~7 seconds).
2. Connect controller via USB.
3. Select 5. Rebuild Database.
4. Wait 5–30 minutes. Fixes filesystem/index corruption.

4. Restore File System (Safe Mode #4)

Same entry → Option 4. Checks/repairs NAND filesystem.

5. Restore Default Settings (Safe Mode #3)

Resets settings only—keeps saves/games. May restore icons.

6. Firmware Reinstall (Safe Mode #6/7)

• #6: Update System Software (via internet/USB).
• #7: Initialize PS3 (DELETES ALL DATA – last resort).

Hardware Suspects

• HDMI cable/port – try another cable/port/TV.
• RSX (GPU) solder fatigue – common on fat PS3 (YLOD precursor). Needs reflow/reball.
• NAND degradation – bad blocks corrupt XMB assets.
